Print and complete WUE form and provide proof of residency (if you are a resident of one of the following states: AK, CA, CO, HI, ID, MT, ND, NM, NV, OR, SD, UT, WA, WY) |

Print and complete New Mexico Waiver and provide proof of residency (if you are a resident of New Mexico) |

Complete and submit the Residence Life Application online (if paying $150 deposit by credit card) |

Print and complete Residence Life Application (if paying $150 deposit by check or money order payable to EAC) Send the Residence Life Application with payment to:
If you do not pay the $150 deposit, a room will not be reserved for you. This Deposit is not included in your scholarship and is your responsibility to pay. |

Print and complete Insurance information form - Include a copy (front and back) of your insurance card |

Schedule a physical exam with your doctor Print and have your doctor complete Physical form provided by EAC |

Print, read and sign Athletic Code of Conduct and Commitment to Excellence form indicating that you understand and agree to abide by this policy. |

Print, read and sign Substance Abuse Education and Testing Program policy - Initial and sign the Consent to Drug Testing and Authorization for Release of Test Results, indicating that you understand and agree to abide by this policy. |

Print, complete and sign Disclosure Form |

Print and complete Student Athlete Information Form |

Take Placement Tests and send results to the Athletic Department |

Have an official copy of your final high school transcripts sent to the Office of Admissions at Eastern Arizona College. This copy must be signed and indicate the date of your high school graduation. |

Have an official copy of college transcripts sent to the Office of Admissions if you have taken ANY college courses. |

Below are links to forms that will need to be completed and returned to the athletic department. These forms are necessary for you to participate in intercollegiate athletics at Eastern Arizona College.
In order to participate, official high school and college transcripts must be received. If you are a new student to EAC, please contact your high school and have official transcripts sent directly to EAC. If you have attended any other college, or taken any college classes, contact that college and have official transcripts sent directly to EAC.
Parents:
The requirements below include an insurance form that must be filled out and signed by you in order for your child to participate.
If your child is under the age of 18 when forms are completed, please sign next to your child on all forms.
